Three months later, the defect returns. What happened in that room was not analysis; it was performance. The team reached consensus because disagreement felt uncomfortable. Janis identified specific symptoms that drive this failure mode. Illusion of invulnerability leads teams to assume their existing processes are robust enough to handle any anomaly. Collective rationalization causes them to dismiss outlier data as measurement error rather than a critical signal. The illusion of unanimity guarantees that silence is interpreted as agreement.
Apply these symptoms to your last FMEA review or management review meeting. Teams convince themselves they are being objective because they are looking at data. But when a group collectively favours one explanation, members unconsciously filter evidence to support it. They are not being deliberately misleading; they are genuinely blind to the disconfirming evidence right in front of them.
Quality teams are uniquely vulnerable to this dynamic. The pressure of impending IATF 16949 or AS9100 audit deadlines creates urgency, which suppresses critical thinking. Teams converge on the first plausible explanation to close the CAPA quickly. The structured discipline required by quality standards is abandoned in favour of social agreement.
Authority and Hierarchy in Problem-Solving
Organizational hierarchy actively suppresses the intelligence available in the room. When a quality director or plant manager leads a problem-solving team and expresses an opinion early, they anchor the entire discussion. The rest of the team aligns with that position. The junior engineer with the contradictory data stays silent.

Technical authority compounds the problem. The Six Sigma Black Belt who ran the design of experiments dominates the discussion. But their expertise in statistical methodology does not mean they see the whole picture. If operators were not included in the experimental design, the human factor is missed entirely. Nobody challenges the methodology because the authority figure knows the statistics.
Excessive team cohesion breeds conformity. Quality teams that work well together develop trust, but they also develop a desire to maintain harmony. This desire overrides the need to find the truth. Teams that like each other tend to avoid productive conflict, which is exactly the friction required to surface hidden variables in complex manufacturing processes.
The Anatomy of a Failed Corrective Action

Someone quickly rationalizes this, suggesting the new die was not qualified properly. The group accepts this explanation and writes the CAPA.

Watch for an absence of minority opinions. In a properly functioning problem-solving team of eight people, you should hear at least two or three alternative perspectives on a complex issue. Uniform agreement is a warning sign. If every person in the room agrees with every conclusion, someone is self-censoring to avoid social friction.
Pay attention to the language of certainty. Phrases like obviously, clearly, and everyone knows are not arguments; they are mechanisms of social pressure. They signal that questioning the conclusion means questioning the competence of the group. When these phrases appear, critical evaluation has stopped.

Ask questions, solicit input from the quietest members, and let others build the case before you weigh in. The leader's most powerful tool is restraint.

Groupthink produces confidently wrong answers, which are more dangerous than honest uncertainty in quality management. When a team reaches false consensus, the CAPA fails to prevent recurrence. The organization implements corrective actions that address the wrong root cause, wasting capital and engineering hours on solutions that change nothing on the shop floor.
Customer trust erodes rapidly. When recurring defects persist despite documented corrective actions, customers conclude the supplier either lacks technical competence or deliberately falsified the closure. This directly impacts supplier scorecards and future contract awards. The CAPA system itself loses credibility with auditors and stakeholders.
Psychological safety deteriorates. When dissenting voices are repeatedly ignored, people stop offering their insights. The pool of available intelligence shrinks with each silenced objection. The organization does not just lose the one insight; it loses the person's willingness to contribute anything in the future. The culture becomes compliant rather than curious.
Organizational learning is actively impaired. The team files a CAPA documenting a false root cause, which becomes organizational memory. Future teams reference this documentation. The error compounds as the organization learns the wrong lessons and encodes them into its knowledge base. Over my career, I can count on one hand the times a team's first hypothesis was correct.
